I/We agree if we sign-up for service to Fibrezone Ltd installing and keeping telecommunications apparatus consisting of, without limitation, the radio Customer Premises Equipment and a communications outlet (the Network) at the above address and to the right of Fibrezone Ltd to inspect, maintain, adjust, repair and/or alter that apparatus in accordance with the Electronic Communications Code set out in Schedule 2 of the Telecommunications Act 1984 (and modified by the Telecommunications Act 2003) as may be amended, modified, replaced, upgraded or re-enacted from time to time on the understanding that FibrezoneLtd will make good any damage which may be done in carrying out such installation, maintenance, adjustment, repair or alteration.

I/we also agree to grant Fibrezone Ltd the right to access such part of the above premises as shall be required to carry out these works and to allow Fibrezonee Ltd or its appointed representatives the right to contact me to arrange an appointment for the installation of related equipment at a mutually convenient time.

I/we consent to Fibrezone Limited and its affiliate ISPs and media service providers using my personal information to contact me from time to time with information on their respective products, offers and services which they may be able to provide through the Network.
